UK finds drug-resistant fungus on 5 patients on a floor of hospital

A sneaky, infectious, drug-resistant fungus called Candida auris has been found on five patients on one floor at the Albert B. Chandler Hospital at the University of Kentucky, UK announced Tuesday.

"All cases have the fungus on their skin, but it is not causing an active infection and there are no active infections with Candida auris at UK HealthCare," the university said. "Our team quickly identified the pathogen and began isolation and disinfection precautions to prevent further transmission."

Citing the Centers for Disease Control and Prevention, UK noted that people "can carry Candida auris on their skin and may not have any symptoms. Often those who get it are already admitted to a health-care facility with another serious illness or condition and may have risk factors such as mechanical ventilation, tracheostomy, invasive medical devices and frequent health care encounters."

The fungus is a yeast that can cause infections in the blood, wounds, the respiratory tract and the urinary tract. It is most likely to infect people who are already sick, but "can be resistant to multiple antimicrobials and is responsible for outbreaks in health-care facilities across the globe," UK noted, concluding:

"As an academic medical center, UK HealthCare is steadfast in the surveillance and identification of infections and the implementation of interventional and prevention strategies to contain and help prevent reoccurrence and transmission of diseases and has a dedicated infection prevention and control team that monitors infections and the possibility of outbreaks that could become a threat to public health."

As a drug-resistant fungal disease, Candida auris, adapts to the warmer climate, cases of it rise; up 214% in Kentucky in 2022

Cases of a potentially fatal fungal disease called Candida auris have increased by 1,200 percent since 2017 and the warmer climate could be to blame, Camille Fassett reports for The Associated Press and Grist.

All of Kentucky's 29 clinical cases of C. auris were reported in 2021 and 2022. However, there was a 214% increase between the years, with seven in 2021 and 22 in 2022, according to the Centers for Disease Control and Prevention

Fungal disease expert Arturo Casadevall, a microbiologist, immunologist and professor at Johns Hopkins University, told Fassett that humans normally have tremendous protection against fungal infections because of our temperature. 

“However, if the world is getting warmer and the fungi begin to adapt to higher temperatures as well, some … are going to reach what I call the ‘temperature barrier’,” he said, referring to the threshold at which mammals’ warm bodies usually protect them from infection.

In other words, Fassett writes, "Over time, humans may lose resistance to these climate-adapting fungi and become more vulnerable to infections. Some researchers think this is what is happening with C. auris." 

And while C. auris is not usually dangerous for healthy people, it can be for older patients, people with pre-existing medical conditions and people in health-care settings. The death rate is estimated to be between 30% and 60%, according to the CDC. 

The pathogen was first identified 14 years ago in Japan and early on most cases in the U.S. were linked to people who had traveled from other places, Meghan Marie Lyman, an epidemiologist for mycotic diseases at the CDC, told Fassett. Now, she said, most cases are acquired locally, generally spreading among patients in health-care settings.

"In the U.S., there were 2,377 confirmed clinical cases diagnosed last year," Fassett reports. She adds, "In the U.S., the most cases last year were found in Nevada and California, but the fungus was identified clinically in patients in 29 states. New York remains a major hot hotspot."

Dr. Luis Ostrosky, a professor of infectious diseases at McGovern Medical School at UTHealth Houston, told Fassett that he thinks C. auris is “kind of our nightmare scenario.”

“It’s a potentially multidrug-resistant pathogen with the ability to spread very efficiently in health care settings,” he said. “We’ve never had a pathogen like this in the fungal-infection area.”

The problem is so bad that the CDC issued a news release in March, calling the fungus "an urgent antimicrobial resistance (AR) threat" and noted that it "spread at an alarming rate in U.S. health-care facilities in 2020-21." 

"CDC has deemed C. auris as an urgent AR threat, because it is often resistant to multiple antifungal drugs, spreads easily in health-care facilities, and can cause severe infections with high death rates," the release says.

More action needed to protect patients from antibiotic-resistant infections, which kill an American every 15 minutes, CDC says

More action is needed to protect Americans from antibiotic-resistant infections, 2.8 million of which occur in the U.S. each year, killing more than 35,000 people, one about every 15 minutes, says the federal Centers for Disease Control and Prevention.

On top of that, 12,800 people died from an infection that is not drug-resistant but is "caused by the same factors that drive antibiotic resistance—antibiotic use and the spread of germs," the CDC said in its report, Antibiotic Resistance Threats in the United States, 2019. "The number of people facing antibiotic resistance is still too high. More action is needed to fully protect people."

Hospitals are a main battleground in the fight. The word "hospital" appears 188 times in the 148-page report, which was issued days after the Leapfrog Group's latest rankings of patient safety at hospitals nationwide, including 50 in Kentucky. The most common grade, on an A-to-F scale, was a C, and the report showed that many hospitals had difficulty dealing with resistant infections.

The report's foreword, by CDC Director Robert Redfield, sums up the history of antibiotics in personal terms: "In March 1942, Mrs. Anne Miller of New Haven, Connecticut, was near death. Infectious germs had made their way into her bloodstream. Desperate to save her, doctors administered an experimental drug: penicillin, which Alexander Fleming discovered 14 years earlier. In just hours, she recovered, becoming the first person in the world to be saved by an antibiotic. Rather than dying in her thirties, Mrs. Miller lived to be 90 years old.

"Today, decades later, germs like the one that infected Mrs. Miller are becoming resistant to antibiotics. You could have one in or on your body right now—a resistant germ that, in the right circumstances, could also infect you. But—unlike the bacteria that threatened Mrs. Miller—the bacteria may be able to avoid the effects of the antibiotics designed to kill them. Unfortunately, like nearly 3 million people across the United States, you or a loved one may face an antibiotic-resistant infection."

The use of antibiotics has a self-defeating element: the growth of genetically different bacteria and fungi that have mutated into forms that are resistant to the antibiotics or picked up resistance traits from other germs through mobile genetic elements. Also, antibiotic use encourages development of Clostridioides difficile, or C. diff., a bacterium "that can cause symptoms ranging from diarrhea to life-threatening inflammation of the colon," says the Mayo Clinic. The CDC report says 223,900 cases of C. diff. occurred in 2017, killing at least 12,800 Americans.

"Dedicated prevention and infection control efforts in the U.S. are working to reduce the number of infections and deaths caused by antibiotic-resistant germs, but . . . CDC is concerned about rising resistant infections in the community, which can put more people at risk, make spread more difficult to identify and contain, and threaten the progress made to protect patients," the CDC said in announcing the report. "The emergence and spread of new forms of resistance remains a concern."

What is to be done? "Public-health prevention programs targeting resistant germs can and have worked to slow spread and save lives, but more needs to be done," the report says. "The U.S. and global community must scale up these effective strategies and develop new strategies to prevent infections and save lives. In the United States, infection prevention activities have proven effective in slowing the spread of resistant germs. This includes:
  • Strategies to decrease spread within healthcare settings (e.g., implementing hand hygiene)
  • Vaccinations
  • Implementing biosecurity measures on farms
  • Responding rapidly to unusual genes and germs when they first appear, keeping new threats from spreading

Use of pacifiers could lead to very sick babies, study finds

Oklahoma State University researchers report that they have found a wide range of disease-causing bacteria, fungus and mold on pacifiers that young children had been using.

In research presented at the American Society for Clinical Pathology, scientists have shown that pacifiers can also grow a slimy coating of bacterial film that alters the normal bacteria in a baby or toddler's mouth, increasing the likelihood of colic and ear infections. It gets worse, according to study author Tom Glass, a professor of forensic science, pathology and dental medicine: The same types of bacteria found on a common pacifier have been linked to cardiovascular disease, metabolic syndrome, allergies, asthma and autoimmune diseases.

HealthDay reporters Barbara Bronson Gray writes that Glass said the problems associated with pacificers are also applicable to any removable orthodontic appliance like retainers, mouth guards and dentures. Glass said his researchers were particular concerned about their findings when they discovered that "many of the bacteria growing from the used pacifiers were resistant to commonly used antibiotics such as penicillin and methicillin. The development of such resistance to certain antibiotics does not cause the organism to be more infectious than other strains that have no antibiotic resistance, but it can make the infection more difficult to treat." Three or four more cases of fungal meningitis found in southern Pennyrile region -- two of those in Todd County

Kentucky now has five confirmed reports of fungal meningitis, and the Kentucky New Era reports that three or four have occurred in the southern Pennyrile region. The New Era's Nick Tabor writes that while no facilities in Kentucky have received the steroid responsible for the outbreak, the Centers for Disease Control confirms that the Saint Thomas Outpatient Neurosurgery Center in Nashville, Tenn., and two separate facilities in Crossville and Knoxville have distributed the steroid in injection form. All five Kentuckians who contracted the illness received medical care in Tennessee, the state reported.

The state’s Department for Public Health was notified this weekend of two cases in Todd County. One is Bill Johnson Sr., whose son ran against Rand Paul for U.S. Senate and Alison Lundergan Grimes for Kentucky Secretary of State. Carolyn Bland, director of the Trigg County Senior Center, was hospitalized Oct. 3 for meningitis, said her daughter, Beth Taylor. Eddie Lovelace, a longtime circuit judge from Clinton County, died of meningitis last month. He was being treated at St. Thomas when he died. 

As of Tuesday, health officials have reported 119 infections and 11 deaths across the nation, and six of those deaths were in Tennessee.

Southern Kentucky circuit judge was probably the first to die in Tenn. from fungal meningitis outbreak stemming from tainted steroids

The first person to die in Tennessee from the nine-state meningitis outbreak liked to bad steroids was most likely Kentucky Circuit Judge Eddie C. Lovelace of Albany, 78, who died at Vanderbilt University Medical Center Sept. 17. John Dreyzehner, Tennessee's commissioner of health, said Friday that the number of Tennesseans affected by the meningitis outbreak has now risen to 29. The number of deaths is unchanged at three. The search for more affected patients will continue "for some time," he said. Investigators haven't found evidence that the clinics or clinicians in Tennessee did anything to cause the outbreak, which has been reported in other states.

In July and August, Lovelace received three rounds of the pain-relieving steroid injections suspected of causing the outbreak of the rare disease, Joyce Lovelace, his wife of 55 years, told Adam Tamburin of The Tennessean. Representatives of the Saint Thomas Hospital Outpatient Neurosurgery Center, where he received the injections, are reported to have called Joyce Lovelace twice after his death to discuss his condition. They did not mention the outbreak, she said, nor have they confirmed that that was his cause of death.

Vanderbilt spokespeople have confirmed to the newspaper that "the first reported casualty of the outbreak was a 78-year-old man who died there on Sept. 17." Doctors told his family that his unexpected death was likely caused by a stroke, which is common among critically ill meningitis patients. His symptoms -- slurred speech, trouble walking and numbness -- are consistent with symptoms of fungal meningitis.

Lovelace had been a circuit court judge for two decades, and commonwealth's attorney and county attorney before that. “He always wanted to be known as a judge who knew the law, and he certainly was.” Joyce Lovelace said. “His career was not over. He had years yet to work.” (Read more) Democratic Gov. Steve Beshear may fill Lovelace's nonpartisan vacancy with state Sen. David Williams of Burkesville, who has been president of the Senate since Republicans took formal control of the chamber in 2000.

UPDATE, Oct. 6: The New York Times reports that nine states have reported meningitis cases connected to the steroids made by a Massachusetts company, which shipped the steroids to 14 other states. "Some doctors and clinics have turned away from major drug manufacturers and have taken their business to so-called compounding pharmacies, like New England Compounding, which mix up batches of drugs on their own, often for much lower prices than major manufacturers charge — and with little of the federal oversight of drug safety and quality that is routine for the big companies."
